ability to customise the close call / request screens in the Screen Designer. Merged

Customer would like the ability to customise the close call / request screens in the Screen Designer. They would like this feature to be present in VSM (core) and also in vFire Officer (which currently uses it's own close call screen) Customer would like to customise the close screens to be more compliant with ITIL
